Minecraft port forward help?

Hello,

I want to portforward my minecraft server, and in the image that is what i put. Last time i successfully portforwarded my server and it worked, 2 months later, im not sure why it doesn't now.

My router is tm unifi rg4332. it worked last time because i put :25565 with my public ip. help. i've added a rule to my firewall. i left my minecraft server.properties "server ip=" blank. and yes i have a static ip address. Also,should i enable dmz host or disable..?
